To help us most effectively investigate your complaint, please provide all the information requested on the OCCC Complaint Form. The complaint form provided here is a printer-friendly PDF file. To open and print the form, you'll need Acrobat Reader. If you do not have Acrobat Reader on your computer, you can download it free.

After you've printed out and filled in the form and assembled any documents related to your complaint (such as contracts, proof of payments, etc.), you may fax the form and your documents to 512.936.7610 or mail them to 2601 N. Lamar Blvd., Austin TX 78705.
